Completing these forms is crucial as they furnish us with essential information required to finalise your financial accounts. To facilitate this process, please download and complete the applicable questionnaire(s) following:
For your convenience, you can fill out the BDS Annual Financial Statements Questionnaire as an online PDF. To access and complete this form, please make sure you have the latest version of Adobe Acrobat Reader, available for free on their website.
To save the form to your computer, right-click on the relevant link and select “save as.” This action will allow you access to the ‘signature’ and ‘submit’ fields. Kindly include individual questionnaires where necessary and attach any supporting documentation when submitting the completed forms. Note that for the e-sign option, Chrome or Microsoft Edge browsers are not supported.
